when you have finished filling it out and click on the "Submit" button;
there is no need to print it and bring this form with you.

If you have already scheduled your new patient exam and have received confirmation of the date and time of your appointment, please print the applicable form(s), including the "Registration" and "Notice of Privacy Practices" forms, fill out completely, and bring with you to your appointment.
Information for Multiple Patients: if you are submitting information for more than one person, please fill out a separate form for each new patient.
